This session will provide an in-depth discussion on the lessons learned from the NIMH sponsored Sequenced Treatment Alternatives to Relieve Depression (STAR*D) trial and an overview of future directions. The session will also provide a discussion on why Practical Clinical Trials involving “real world” patients, such as STAR*D, are more likely to provide results that can be immediately utilized in clinical practice. The presentation will also present a model for possible biomarker studies in depression.Educational Goal:
The goal of this session is to provide attendees with an overview of the currently available literature for major depressive disorder (MDD), with a particular emphasis on the STAR*D trial and lessons learned. In addition, attendees will be able to discuss the potential role of biomarkers in the selecting treatments for MDD.Learning Objectives:
- Identify the gap between efficacy trials and “real world” patients and the implications for clinical practice.
- Review the lessons learned from STAR*D, and how they translate to clinical practice.

This session will provide a thorough and engaging discussion about the relative efficacy and tolerability profiles of the typical antipsychotic and the atypical antipsychotics, in the context of individualizing treatments for patients with schizophrenia.The contemporary treatment of schizophrenia has (paradoxically) become increasingly complex with the greater availability and choice among antipsychotic medications. At the same time, there is still substantial debate and contention, fuelled by recent large pragmatic trials in schizophrenia, that the balance of efficacy and tolerability considerations remains complex and as yet inadequately predicted for the individual therapeutic context. Incorporating evidence from various sources on efficacy, safety, and tolerability, to develop effective and individualized strategies improving therapeutic outcomes are key considerations. To address these aspects, we will engage in a lively debate-designed to also engage our audience. This format for this interactive session will be designed to provide a review of the available literature followed by a debate format between two well known clinical researchers regarding efficacy vs. tolerability of antipsychotics in the treatment of schizophrenia.
Educational Goal
The goal of this session is to provide the attendee with the background needed to adequately weigh the risks and benefits associated with antipsychotic medications. As a result, he or she will be able to make evidence-based, balanced clinical decisions regarding antipsychotic regimens during the acute and maintenance treatment phases for patients with schizophrenia.
Learning Objectives- Identify the available literature regarding and differentiate the relative efficacy of typical and atypical antipsychotics used in patients with schizophrenia during the acute and maintenance phases of treatment.
- Identify the available literature regarding and differentiate the relative tolerability profiles of typical and atypical antipsychotics used in patients with schizophrenia during the acute and maintenance phases of treatment (considering morbidity and mortality, cost, and patient preference).
- Compare and contrast the risk-benefit ratio of typical and atypical antipsychotics in the treatment of schizophrenia.
- Discuss the importance of efficacy versus tolerability in the clinical decision making process for the individualized treatment of schizophrenia during the acute and maintenance phases.
