Endorsement of Medical Home Model of Care

Leadership for Medication Management (LMM - formerly the Pharmacist Provider Coalition) is a coalition of pharmacy organizations that continue to work together to secure direct pharmacist reimbursement for cognitive and clinical services. Representatives from LMM have been in discussion with an organization called PCPCC (Patient-Centered Primary Care Collaborative, www.PCPCC.net) to assure the inclusion of pharmacist clinical services in the medical home model of care. PCPCC includes a variety of different organizations in its membership including labor unions, medical associations, health care organizations, governmental entities, advocacy groups and pharmaceutical companies.

The medical home model of care is one of the models being proposed for health care reform. Attached please find a white paper developed by LMM which details this model and outlines principles that must be addressed to assure pharmacist inclusion. We are pleased to announce that the Board of CPNP has voted to endorse this model and
white paper.
