The attached file is a template in the computerized pt record at the Battle Creek VAMC. It is used by pharmacists doing chart review in the Nursing Home Care Unit at this facility. A number of the fields in the template are populated automatically by the computer. Since this is a government institution, it is my belief that this document is in the public domain.
