The Geriatric Psychopharmacotherapy Community is hosting a webinar round table. During the webinar we will hear from Caitlin Dirvonas, PharmD, BCPS, who will present on a pharmacist’s role in an interdisciplinary geriatric behavioral health assessment clinic. Dr. Dirvonas is a PGY2 psychiatric pharmacy resident at the VA – Tennessee Valley Healthcare System. We will also hear from Danielle Fixen, PharmD, BCPS, who will speak on transitional care management with a focus on high risk medications in older adults and medication de-prescribing. Dr. Fixen serves as a clinical pharmacy specialist for the Seniors Clinic at University of Colorado Hospital. This clinic is a primary care site for patients older than 70 years of age. Clinical pharmacy services provided for patients include transitions of care, chronic care management, pharmacotherapy consults, and drug information.
