Clinical pharmacy services to be discussed include (but are not limited to) the following disease states/areas of interest: Epilepsy and other seizure disorders, multiple sclerosis, traumatic brain injury, infusion center, migraine, and neuromuscular disorders. All meeting attendees are welcome, even if you do not currently work within the neurology community.

Five Years in the Making: The Pharmacists’ Patient Care Process (led by the Patient Care Practice Management Community)
It was almost five years ago when the Joint Commission of Pharmacy Practitioners released the Pharmacists’ Patient Care Process (PCPP) to address the need to establish a consistent process of providing patient care across the profession. Using the principles of evidence-based medicine, pharmacists are encouraged to use a 5-step, cyclic process for care delivery: 1) Collect, 2) Assess, 3) Plan, 4) Implement, and 5) Follow up. While many of us are aware of this process and likely use some of the guiding principles of the PPCP, it can be challenging to apply the PPCP to all practice settings in its entirety. However, given the continued shift to outcomes-based payment models across the healthcare system, the importance of measuring outcomes of pharmacists’ services in a meaningful way grows, and following the PCPP is one of the ways to ensure consistency across our profession. Participants will engage in guided discussions regarding the history and potential value of the PPCP, steps for implementing the PPCP across all settings and strategies of addressing challenges, utilization of the PPCP within Comprehensive Medication Management, and methods of documenting outcomes related to pharmacists’ services. Residents and new practitioners are welcome!

Shared Strategies in Group Education for Substance Use Disorders (co-led by the Patient Medication Education Group Community and the Substance Use Disorders Community)
This Idea Exchange will be focused on sharing strategies and examples that engage patients in group education related to substance use disorders. The plan is to discuss various formats, creative ideas, and worthwhile topics and concepts for group education for patients with substance use disorders. We will also discuss how these strategies may change in various settings (outpatient groups vs. inpatient residential vs. inpatient psychiatric unit). The facilitators will come prepared with their own ideas and examples of successful group education, and participants will be encouraged to share their ideas and examples. The goal after this Idea Exchange is to generate a selection of group education templates and ideas to share with all CPNP communities and members.

Chronic pain within addiction medicine requires a multidisciplinary approach to provide various perspectives and treatment strategies. Conversations with patients about chronic pain and opioid use can be challenging and involve motivational interviewing. This idea exchange will provide pharmacist attendees an opportunity to have a discussion with a psychologist specialized in working with patients who have substance use disorders and concomitant chronic pain. After a brief presentation/discussion from the facilitators, attendees will be prompted to discuss barriers, strategies, and ideas about how to have successful conversations with patients related to opioids and chronic pain.

Mental health and wellness of pharmacists are on the radar of professional pharmacy organizations. However, published literature on effective strategies for improving mental well-being is sparse in pharmacy education literature. Participants will be asked to share strategies they may have implemented or are considering implementing ranging from preventative, crisis response, and educational programs.
