The CPNP Communities Lunch and Learn Webinar Series provides a weekly opportunity for CPNP members to learn and grow together while focusing on timely topics selected by the Community Leaders. This member-driven initiative does not provide ACPE credit. Instead, the leaders are able to focus on late-breaking items, fostering discussion on the calls, and generally being responsive to the questions and issues emerging in real practice.

As a CPNP member, you are welcome to register for as many of the webinars as you like for free. Recordings will be made available for most of the webinars, and presentations will also be provided for download as appropriate.

We will also hear from Danielle Fixen, PharmD, BCPS, who will speak on transitional care management with a focus on high risk medications in older adults and medication de-prescribing. Dr. Fixen serves as a clinical pharmacy specialist for the Seniors Clinic at University of Colorado Hospital. This clinic is a primary care site for patients older than 70 years of age. Clinical pharmacy services provided for patients include transitions of care, chronic care management, pharmacotherapy consults, and drug information.

We will discuss what students learned in the classroom and the activities they were asked to do outside the classroom including motivational interviewing and attending a 12-step meeting. We will also discuss the findings from data collected from this course. Speaker Andrew Muzyk, PharmD, was the co-lead on this project and his team has submitted manuscripts on this project to Academic Medicine, Teaching and Learning in Medicine, and Substance Abuse.
